Are you 18 years of age or OLDER?

Please bring a valid, goverment-issued photo ID showing your name, photo, and date of birth.

Accepted Examples:

  • Driver’s License

  • State ID

  • Passport

  • Military ID

Are you UNDER 18 years of age?

A parent or legal guardian must be present for the appointment.

Please bring:

  • The minor’s birth certificate

  • The minor’s photo ID or school ID, if available

  • Parent/guardian’s valid government-issued photo ID

  • Any guardianship documents if the accompanying adult is not listed on the birth certificate

IMPORTANT

Names on the documents must match, and all required documentation must be brought to the appointment. If the required ID/documentation is not available, we may not be able to perform the piercing.
